PHYSICAL THERAPIST FOR HOME VISITS

Aventas Home Health is excited to be looking for an additional Experienced PHYSICAL THERAPIST  to provide HOME HEALTH VISITS to our clients on a per diem basis. 

Work as much as you want- supplement a full time job, or build up to full time work with an experienced supportive team!

The Physical Therapist will report to the Director of Home Health Services and provides skilled Physical Therapy services on an intermittent basis to clients in their homes in accordance with the physicians orders.

Responsibilities Include:

• Conducts initial and ongoing comprehensive assessments of the client's needs, including OASIS Assessments and teaching for family and caregivers
• Assists the physician in evaluating the clients level of function by applying diagnostic and prognostic procedures
• Establishes a treatment plan in consultation with physician, and revises it as necessary
• Assures that the physicians orders are appropriate and discusses necessary changes
• Assists in development and implementation of the interdisciplinary care plan to include Physical Therapy
• Evaluates the client's home situation to determine what instruction will be required and what assistance is available to progress to goals and return to prior level of functioning

Experience requirements

Must be a licensed PHYSICAL THERAPIST in the state of Arizona

Must have at least 1 year of supervised or administrative experience in home health care or related health care experience

Graduate of a Physical Therapy School Approved by the Council of Medical Education of the A.M.A.in collaboration with the AMPTA
