Our nurses are simply the best! Aventas Home Health is currently hiring experienced RN’s and LPNS for intermittent home health care. We provide flexible PRN visit schedules. This position responsibilities include: Oasis start of cares including in home client assessments, reviewing patient medical history and developing personalized care plans that support quality measures. Teach & educate patients to maximize their goals and outcomes. Observe & report signs, symptoms and changes in condition with physicians. Case management and discharge planning. Education requirements

Graduation from an Accredited School of Nursing

Experience requirements

Must be a Registered Nurse or Licensed Practical Nurse in the state of Arizona

Must have at least 1 year of experience in home health care or 2 years experience in an institutional setting

Qualifications
